Instructions for Submitting a Damage Report
Please use this form to report any damage to equipment, materials, or locations.
Providing complete and accurate information allows us to process your request quickly and efficiently.
Please provide the following information:
- Title – short description of the damage (e.g. “Damaged display – Store Berlin”)
- Name and email – for follow-up and status updates
- Select type – please choose between Repair or Replacement
- Location – affected store or site
- Description – detailed explanation of the damage
- Attachments – photos or relevant documents (if available)
Important:
- Damage must be reported immediately after it is identified.
- Please include clear and relevant photos of the damage.
- A complete and precise description significantly speeds up processing.
- In urgent cases, please also contact your responsible contact person and submit this report in parallel.
